A Clinical Study of Tuberculous Cervical Lymphadenitis


Abstract
A clinical review for 89 cases of tuberculous cervical lymphadenitis was made during past 5 years from May 1987 to April 1992 at the Department of Surgery, Hanil Hospital.
@ES The results were obtained as follows;
@EN 1) The age of the highest incidence was 20~29 years old(36.0%) and females predominated over males by 2.6 : 1.
2) the duration of disease was less than 1 months in 59.6%(53 cases), 1 months to 3 months in 14.6%(13 cases).
3) The frequency of location was as follows; 93.3%(83 cases) of the cases was unilateral. Of 83 cases in which were unilateral, 54.0%(48 cases) was right side and 39.3%(35 cases) left side. The anatomical location was as follows: posterior
cervical
lymph node 34.8%(31 cases), supraclayicular lymph node 33.7%(30 cases), combined type 20.2%(18 cases).
4) Single mass was seen in 25.8%(23 cases) and multiple masses in 42.7%(38 cases), and cold abscess I 27.0%(24 cases).
5) The clinical manifestation was follows; painless swelling of lymph node in 67.4%(60 cases), painful swelling of lymph node in 32.6%(29 cases), tenderness in 47.2%(42 cases), and systemic symptom included cough with sputum and fever in 10.1%(9
cases)
each other, fatigue in 5.6%(5 cases).
6) Seasonal distribution was as follows; spring 41.6%(37 cases), winter k22.5%(20 cases). Summer 20.2%(18 cases), autumn 15.7%(14 cases).
7) the incidence of associated tuberculosis was 56%, and its frequency was pulmonary tuber culosis (33.7%), axillay tuberculosis(9.0%), inguinal tuberculosis(5.6%), intestinal tuberculosis(2.2%) in order.
8) All cases were treated with antituberculous chemotherapy. And radical lymphadenectomy was done in 43.8%(39 cases), incision and drainage or fistulectomy in 29.2%(26 cases).
9) The recurrence was in 29.2%(7/24 cases) after chemotherapy only and 38.4%(10/26 cases) after I & D with chemotherapy, and 12.8%(5/39 cases) after total excision with chemotherapy.
